Payment Options: Payment At Time Of Service: We accept cash and check. By paying in full on the day of service, we will give you a 5% reduction. This is our way of saying Thank-you for allowing us to provide your oral health care needs. We also accept major credit cards and Care Credit at the time of the appointment. If treatment will be extended for several visits, you may pay a portion of the total fee at each visit, with payment in full at the last visit. |

| Care Credit: Care Credit is a health care/dental care credit card that can only be used for health or dental related charges. It allows interest free or low interest on services rendered. Please ask our receptionist for further details. | |
| Patients With Insurance: We are pleased you have selected us to provide your dental care. As a courtesy to you, if you have dental insurance, Family Dentistry will file the claim for you. Please be aware, the dental insurance is a contract between you and the insurance company. Any portion your insurance does not cover, you are responsible for. We ask that you pay your portion at the time services are rendered. | |
